WINTER HAVEN HOSPITAL BENEFITS 2010

Enrollment eligibility for Health, Life, Dental and Vision Insurance:  Employees must be assigned full time with a .8, .9 or 1.0 FTE (Pay-in-Lieu of benefits employees who opt out of benefits are not eligible for health, life, dental, and vision benefits).

Enrollment into group and voluntary insurance plans:  Within the first 90 days of employment, new employees who meet eligibility requirements may elect group coverage for health, life, dental and vision insurance or Pay-in-lieu of benefits within the first 90 days of employment. . Other benefits such as flexible spending, pay-in-lieu of PTO (non-exempt only) and Voluntary benefits (see below) may be elected at this time as well. Employees who do not make an election during this period may enroll during our open enrollment period (usually October).

Coverage begins 1st of month following 63 days active employment:  Payroll deductions start one month prior to effective date for Health & Dental coverage and therefore, employees are encouraged to enroll within the first 30 days to avoid “catch-up” deductions. Life and Vision deductions start the month the insurance becomes effective.

Section 125:Our Group Benefits are considered as IRS Code Section 125 paid with pre-tax dollars. This means that premiums are paid prior to taxes being deducted from employees pay, therefore the employee is not paying taxes on these premiums. Section 125 employers must follow guidelines (refer to the HR policy manual) to stay in compliance. Listed below are the benefits subject to Section 125: Health, Dental, Vision, Flexible Spending Accounts and all benefits offered by Aflac.

HEALTH – Winter Haven Hospital has a self-insured health plan with UMR contracted as the Third Party Administrator.  Health plan premiums are reasonable with WHH supporting approximately 80% of the premium.

WHH offers 2 health plans:

  • Traditional (co-pays and low deductible)
  • High Deductible PPO with HRA

TERM LIFE INSURANCE – Offered through Lincoln Financial: Eligible employees are provided term life insurance which is one and one half (1 ½) times annual base salary. Premiums are paid by Winter Haven Hospital.

DENTAL INSURANCE – offered through Lincoln Financial; offers a high and low plan option. These plans cover Preventive/Diagnostic, Restorative/Surgical, Prosthodontia, and Orthodontic Procedures.
Employees have coverage in network as well as out of network.

VISION INSURANCE –offered through EyeMed. It provides coverage for eye exams, lenses/ frames/contacts per schedule

PAY-IN-LIEU OF BENEFITS - This program allows employees who are assigned to a .8 FTE position (64 hrs per pay period) or higher to opt out of health, dental, vision & life and receive $46.15 per pay period in lieu of their benefits.

NOTE: All covered dependents 19 to 25 years of age that attend a secondary school, college or university must be enrolled in our coverage and provide student verification of full time status every semester or quarter to Employee Benefits.  If verification is not received dependents will be disenrolled from the plans.

FLEXIBLE SPENDING ACCOUNTS—(enrollment for .5 FTE & greater) During the annual Open Enrollment, employees’ have the options to have Pre-tax money from next year’s paychecks withheld for Medical and/or Dependent Care Expenses. New hires can enroll for the remainder of the current year. Debit card available for the Medical FSA.

PENSION AND RETIREMENT SAVINGS PLAN

Eligible employees must be 21 years old, complete 1 year of service and work 1,000 hours in plan year.

PENSION:  Winter Haven Hospital has a defined benefit pension plan and eligible employees receive an employer contribution based on the years of vested service an employee has with the plan.  Contributions are 3.25% of annual base salary for 1-3 years of service, 3.75% for 3 -10 years of service and then 5% of annual base salary for 10+ years of service. Employees are 100% vested in the pension program after completing 3 years of vested service.

RETIREMENT SAVINGS PLAN – Offered through Valic:  Employees may contribute in a Retirement Savings Plan and the hospital will match up to 2% for the first 6% (annual base salary) contributed by employee.  Employees are 100% vested in the AIG program after completing 5 years of vested service.  Prior to completion of 5 years vested service, there is a cliff vesting schedule for the employer match.

The Valic Representative schedules weekly appointments at all facilities. 

Voluntary Benefits

Eligibility:

  1. Completion 90 day new hire probation period
  2. Assigned/work minimum 40 hours per pay period (.5 FTE)

CANCER, SPECIFIED HEALTH EVENT, HOSPITAL and ACCIDENT INSURANCE – Offered through Aflac  Premiums based on coverage you select

DISABILITY – (Short Term & Long Term) – Offered through Lincoln Financial.  Must not have other disability insurance.

LONG TERM CARE – Offered through Unum Provident

VOLUNTARY LIFE INSURANCE* – Lincoln Financial. Employees may purchase Life insurance coverage in $10,000 increments up to a maximum limit. Spouse and children’s coverage is available, sometimes medical insurability is required for guarantee issue. Costs will vary due to age and coverage selected.

ADDITIONAL BENEFITS

PTO (Paid Time Off) is inclusive of holidays, vacation, and sick days allowing for from 26 days off to a maximum of 37 days per year for full time employees. PTO is earned based on hours worked, up to 40 per week, for eligible employees. The accrual of time begins with employment for employees assigned to 32 hours per week or greater and is available for use after the 90 day new hire probationary period. Any hours remaining when employment ends are paid out according to Policy.

The Pay-in-Lieu of Paid Time Off (PTO) allows employees who are assigned to a .8 FTE position (64 hrs per pay period) to decline Paid-Time-Off (PTO) and receive 10% of their current base salary in lieu of this benefit. Exempt employees are not eligible for this benefit.

An Employee Assistance Program (EAP) is available to employees and their family members.

Free parking.

Direct deposit is available through payroll for most financial institutions. Pay Periods are bi-weekly.

Differentials are paid for shifts when at least 4 hours are worked between 2:30 p.m. and 7:30 a.m., weekend differentials are paid when at least 4 hours are worked between 10:30 p.m. Friday and 7:30 a.m. Monday.

Premium pay (time and one/half) is paid for working a holiday. Holidays are New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day.

Tuition Reimbursement is available after completion of the New Hire Probationary period for programs approved by Department Manager and Administration up to $1500 per fiscal year.

Most positions have an annual merit review based upon individual performance standards.

Ongoing market studies of salaries are utilized by Human Resources Compensation to ensure salary competitiveness.

PRN status usually involves a higher pay rate for employment without guaranteed hours and benefits.

The Education Department offers many programs for personal and professional development. Healthcare professionals requiring continuing education are able to attend programs for credit hours, usually free of charge.

General orientation is provided on the first day of employment, which includes Organization Safety and Health Administration (OSHA) regulations and Joint Commission (JC) standards. Department orientation provides an in depth assessment of competencies and required review and training to assure the new employee has the skills necessary to care for the specific population served and/or provide quality service to our customers.

A specific Nursing Orientation is held twice a month to ensure competencies. A Graduate Nursing Orientation is held twice a year, which includes a National Council Licensure Examination (NCLEX) review.

Leave of Absence - Employees going on a leave of absence should check with the Benefits Office concerning continuation of benefits.  Proper procedures should be followed to continue benefits.
